Case Manager - Multiple Positions

The Case Manager performs non-clinical case management with the responsibility for tracking medical status of ALL assigned cases, or Soldiers as required.  This position shall not perform inherently governmental functions (i.e., representation of government, decision authority, etc.)  Duties include:
  • Responsible for medical charting in eCase to ensure all administrative charting in eCase is completed to include any contact with SM (via email, voice call, or personal interview), coordination with Soldier for treatment plans with appropriate medical facilities.  Ensure all administrative charting in eCase is completed within 24 hrs of contact. Request profile exams/evaluations and audiology exams at the physical exam station. Coordinate Soldier treatment plans with appropriate medical facilities. Submit case processing, medical documentation, and profile concerns to military/contacted provider and track medical appointments by communication with the unit, and/or Solider to ensure Soldier attendance and submission of follow up documentation.
  • Maintain automated referrals indicated in MEDCHART. Submit case processing, medical documentation, and profile concerns to military/contracted providers. Attend PHA events as determined by the needs of the state and the individual units.  Ensure those Soldiers referred by a provider are followed by the case management team. Manage the Soldier’s case from the identification of the condition to when the Soldier is either: Considered medically ready or Administratively separated from the military. 
  • Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40-501/502 and other applicable guidance.  Ensure all medical documentation received for any Soldier and/or their medical team is received and placed in the appropriate electronic record, to include all lab work, and referrals. Maintain soldier medical records in accordance with Army Regulation (AR) 40- 66. Maintain the Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines. Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40-501/502 and other applicable guidance. 100% of the time.  Obtain a release of information from the Soldier. 
  • Educate Soldiers on resources available to improve medical and dental readiness. Maintain the Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.  Review, upload, and track health assessment (PHA, SRP, Specialty Physicals), lab results, and contact SM and educate on lifestyle modifications. Upload medical documents of case-managed Soldiers into Health Readiness Record (HRR). Stamp all medical documents for verification after uploading. Index uploaded medical documents. MODS entry of immunizations, the designation of pregnancy, eye exams, medical warning tags. 
  • Provide all medical documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the contractor to the government. Copy of each supporting transfer document (DA form 200) shall be provided to the COR at the end of each month no later than 7 calendar days at the end of the month.  Ensure all administrative charting in eCase is completed. 
  • Track medical appointments by communication with unit, and/or Solider to ensure Soldier attendance and submission of follow up documentation. Responsible for medical charting in eCase, to include any contact with SM (via email, voice call, or personal interview), coordination with Soldier for treatment plans with appropriate medical facilities. Ensure all administrative charting in eCase is completed within 24 hrs of contact. Communicate with delegated government personnel to track compliance versus noncompliance. Ensure Soldiers with profiles are case managed in the eCase module.
Education & Licensing:  
Associate degree RN at a minimum, Bachelor’s degree nurse preferred. LPNs without a degree may seek a waiver for the educational requirements, but this will be based on previous Army experience. The nursing license must be current and in good standing. Preferred experience within the National Guard or the Army as a Service Member. 

Skills:
Must be proficient with MS office suite and general administrative duties.
